An acknowledgement by the authors was omitted by the publisher in the November/December 2006 issue of this Journal (Dis Mon 2006;52:407-458). The following excerpt should have appeared in each article, “The views expressed in this article are those of the authors and do not reflect the official policy or position of the Department of the Navy, Department of Defense, or the United States Government.”

Also on the title page of the issue [(Dis Mon 2006;52:403) blind folio], the information on the fourth author in the second column appears incorrectly. The correct information is below:
